TERMS AND CONDITIONS
This service is a screening and assessment service only and does not constitute a definitive diagnosis or replace consultation with a GP or specialist.
The consultation, clinical assessment, and image capture are provided under the responsibility of Evans Pharmacy pharmacists. Although consultations may take place within a Second Look Medical Group clinic, Second Look Medical Group holds no responsibility for this part of the service.
Dermatology review is conducted by an independent specialist based on the images and information submitted. Accuracy is dependent on the quality of images and clinical information provided.
At the point of receiving results, responsibility for ongoing care transfers either to the client’s GP (if referral is advised) or to Second Look Medical Group (if further treatment is appropriate). Evans Pharmacy holds no responsibility for ongoing management following results.
Clients are responsible for monitoring their skin and seeking medical advice for any changes, even after receiving results from this service.
This service is not suitable for urgent, rapidly changing, bleeding, painful, or highly suspicious lesions. In such cases, clients should seek immediate medical attention via their GP or appropriate healthcare service.
By proceeding with the service, you consent to clinical images and relevant information being securely shared with a third-party dermatology provider for the purpose of assessment.
Evans Pharmacy and Second Look Medical Group are not responsible for delays in results caused by third-party providers.
Failure to attend an appointment or late cancellation may result in loss of the consultation fee.
Clients under the age of 18 must be accompanied by a parent or legal guardian, who must provide consent for the consultation and image submission.
Treatment recommendations following results are optional, and all decisions remain the responsibility of the client.
